Acknowledge, quantify and coordinate physician and health care provider charity care, and spread that care as equitably as possible.

Making it Easier

Make it easier for physicians who are already caring for the uninsured to better manage and coordinate care.

Coordinates Medical Care

Navigates care for Collin County residents who earn 200% or less of the federal poverty levels.

Client Goals

Encourages clients to create goals of good health and self-sufficiency.

Medical Home

Each PACC patient is provided access to a medical home and access to routine and preventative healthcare.

Specialty Care

Each PACC patient is provided access to specialty care and access to routine and preventative healthcare.

Ancillary/Diagnostic Services

Each PACC patient is provided access to ancillary/diagnostic services.

Medications

Each PACC patient is provided up to four generic medications per month, plus diabetic supplies.

Health Education

PACC provides health education classes for clients such as, basic healthy living, diabetes education, lifestyle changes.

Interpretation

PACC employs two bi-lingual staff members assist our Spanish speaking clients.

Transportation Support

To overcome a barrier to accessing healthcare, PACC provides transportation support to clients who have difficulty getting to and from appointments.

Specialty Care

Each PACC patient is provided access to specialty care and access to routine and preventative healthcare.

Community Partnerships

PACC empowers clients to participate in their care and coordinates with other local, state and federal resources to promote our community’s overall wellbeing.

Navigation Services

Our Patient Navigator provides administrative and management support, such as, appointment reminders, transportation coordination, access to free or reduced cost medications, etc., which enhances benefits and fosters physician participation.
